2. Documents (formats) to be submitted when making a "request for disclosure"
When making a disclosure request, please download the application form below, fill in all the required information, and mail it along with documents to verify your identity.

a) Fill in the required information on the "Personal Information Disclosure Application Form" and affix your official seal
b) A copy of your identity document (driver's license, passport, health insurance card, etc.)
c) The person’s personal seal certificate - * In the case of a request made by a representative, in addition to the above, the following documents must be submitted:

- ・Power of attorney from the person in question
- (Any form. However, the applicant's signature, official seal, address, and name and address of the representative are required.)

3. Regarding the method of responding to requests for disclosure, fees, and collection methods
To request disclosure, please mail the above "Personal Information Disclosure Application Form" to the CRM Department. Please note that we will respond to requests for disclosure by mail, so please enclose a 1,000 yen stamp as a handling fee with the above "Personal Information Disclosure Application Form" and mail it to the CRM Department.
4. Method of responding in writing to a "request for disclosure"
We will respond to the applicant by personal mail to the address stated on the application form.
5. "Purpose of Use" of Personal Information Obtained in Relation to Request for Disclosure
Personal information obtained in response to a request for disclosure will be handled only to the extent necessary to fulfill the request. Documents submitted will be discarded within six months after the response to the request for disclosure has been completed.
*Reasons for non-disclosure of retained personal data
In the following cases, we will not disclose the information. If we decide not to disclose the information, we will notify you of that decision and the reason. In addition, we will charge a designated fee for non-disclosure.
- ・When the identity of the person cannot be confirmed, such as when the address stated in the application form, the address stated in the identity verification document, and the address registered with our company do not match.
- When an application is made by a proxy, the authority of the proxy cannot be confirmed.
- ・If there is a problem with the required application documents
- ・ When the subject of the request for disclosure does not fall under "retained personal data"
- ・When there is a risk of harm to the life, body, property or other rights and interests of the person or a third party
- ・When there is a risk of causing significant disruption to the proper performance of our business
- ・When it would violate other laws and regulations.
2. Handling complaints, consultations and inquiries regarding personal information
(1) If there is a request from the person (including the agent) regarding notification of the purpose of use, disclosure, correction (change), or suspension of use and provision of personal information held by the company, the company will promptly respond after verifying the identity of the person. The company will also respond to questions and inquiries regarding the handling of personal information by the company. However, regarding deletion or erasure, we may not be able to comply with your request if it conflicts with legal storage obligations.
(2) Complaints, consultations and inquiries regarding our personal information should be directed to the following office. Depending on the content of the inquiry, we may ask you to submit necessary documents and answer questions. If we notify you of the purpose of use of your retained personal data or disclose your request, we will reply by mail addressed to you, and a handling fee of 1,000 yen (tax included) will be charged.
